8. Review the note regarding additional user accounts; remember that existing users can later add others if they have the privilege.
9. Acknowledge your understanding of the rules and policies by signing and dating where indicated. Ensure all information is accurate before finalizing.
10. Once all fields are completed, save changes, download, print, or share the form as required.
