Item Scores The conners rating consists of a number of multiple-choice questions (59 questions for the Teacher rated conners, 80 the Parent rated conners), these are known as items. For each item there are four multiple choice answers scored 0 to 3. So each item can have a score of 0 – 3.

The SFIRS is principally intended for use in clinical assessment of ADHD severity; therefore, responses use a 5-point Likert scale (0=never, 1=occasionally, 2=sometimes, 3=often, 4=always). The items are all positively scored. Higher scores indicate more serious symptoms or more severe impairment.

Depending on the question, responses are either scored as 0 or 1. On items 1-3, 9, 12, 16, and 18 ratings of sometimes, often, or very often are assigned one point (ratings of never or rarely are assigned zero points).

The Conners Abbreviated Symptom Questionnaire may be the most effective diagnostic tool for ADHD because of its brevity and high diagnostic accuracy, and the CBCL-AP could be used for more comprehensive assessments.

The ADHD Rating Scale-IV (ADHD-RS) rates the 18 symptoms of ADHD as defined in the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) on a four-point (0 to 3) scale from “never” to “always” and on a number of related scales. (Total score range is 0 to 54.)

For each item, or behavior, you note the frequency from not at all to very much. These responses are ranked on a scale of 0 to 3. Once you add up the scores for each section, you divide the number by 9 to determine an average. On the Snap-IV scale, teachers can rate a child who scores above 2.56 as inattentive.

On the ADHD-RS-5 scale, the individual rates the frequency of each symptom or behavior over the preceding week on a 4-point Likert scale ranging from 0 (no or rare symptoms) to 3 (severe or frequent symptoms). The sum of scores for the 18 items provides the total score (ranging between 0 and 54).
