- Designate your primary and contingent beneficiaries by entering their names, relationships, social security numbers, and percentage of account balance. Remember, you can only designate one primary and one contingent beneficiary on this form without additional forms.
- Review the Participation Agreement, ensuring that you understand any withdrawal restrictions and other important terms.
- Finally, sign and date the form. Remember that a handwritten signature is required. Once completed, forward the form to Empower Retirement via the address provided.
