The DD Form 2656-7 is used to establish the Survivor Benefit Plan after the passing of an active, reserve, or retired military service member. This checklist is designed for use by the spouse (widow or widower), eligible former spouse, or child of a deceased military member/retiree.

Active Duty W-2 Earnings Statements Active duty members can visit the DFAS tax statement website to request or view their tax statements. Those with limited internet access can call DFAS at 800-321-1080 M-F, 8 a.m. to 5 p.m. ET to have a paper copy of their active duty W-2 reissued to them.

Military retirees and annuitants receive a 1099-R tax statement either electronically via myPay or as a paper copy in the mail each year.

1099-R tax forms are mailed by the end of January each year. If you recently moved, be sure to update your mailing address. You can access and download your tax statements online by logging in to myCalPERS. To request a duplicate or replacement 1099-R tax form by mail, contact us.

Filing taxes after you've separated If you've separated within 13 months, you can login to your myPay account to get your W2. If you can't access your myPay account, you can submit a tax statement request via askDFAS and we'll mail you a hard copy.

In the event you lose your Form W–2, or require a duplicate copy you can download a copy from Cal Employee Connect under the "W-2" tab. You also can request one from the State Controller's Office.

You can also use Form 4506-T, Request for Transcript of Tax Return. Check the box for Form W-2, specify which tax year(s) you need, and mail or fax the completed form. Most requests will be processed within 10 business days from the IRS received date.
