10. Fill out the Adverse Events/New Medical Diagnoses section by indicating if any new illnesses or adverse events have occurred. Provide additional details for each incident.
11. If applicable, complete the Adverse Events of Special Interest based on the prior responses. Ensure any necessary additional forms are noted.
