8. Explain the direct relevance of your conference to the mission of the AHA.
9. Provide detailed information about any meetings on this subject that have been held in the last three years, including outcomes and sponsoring organizations.
10. Describe how the impact of the conference will be measured effectively.
11. Indicate whether there is a representative from the AHA on the planning committee, and if so, provide their contact information.
12. If this is a recurring conference, please note the year and the number of attendees for the last two events.
13. If you are applying for financial co-sponsorship, specify the amount of funds requested and how those funds will be used.
14. Select up to three AHA Councils for consideration for your request. If selecting more than three, include a written justification.
15. Attach the required documents, including the preliminary program, planning committee roster, faculty roster, and your organization’s conflict of interest policy.
16. Sign and date the application form. Remember that applications that are not signed will not be considered.
17. Finally, submit your completed application via email to the designated AHA email address. Note that hard copies will not be accepted.
