9. Fill out the academic record carefully, listing your certificates and degrees along with your major subjects and percentage of marks obtained. Only use digits for percentage values to ensure proper processing.
10. Affirm the undertaking statement at the bottom of the form, ensuring you understand the implications of providing false information. Sign and date the form accordingly.
11. Once all sections are completed, review the form for accuracy. Ensure you have attached the necessary documents, including the original bank deposit slip, photographs, and a photocopy of your CNIC.
