4. Provide the date of birth for the animal using the format: month, day, year.
5. Complete the sire and dam sections with their respective details, if available.
6. Indicate where the animal was purchased from, including the name, city, and state of the seller.
7. Enter the phone number of the seller and the date of purchase using the format: month, day, year.
8. Fill in your information as the applicant, including your name, age, address, and phone number.
9. Read and acknowledge the certification statement by signing and dating the application.
10. Ensure that the seller or breeder signs and dates the form, confirming the animal's identity.
11. Obtain the signature from a County 4-H or Extension Agent or FFA Advisor, confirming the animal meets the breed requirements.
12. Prepare two digital or scanned photographs, one of each side of the animal, and email them to the provided address. If unable to send digital images, attach physical photographs (no larger than 4” x 6” and not Polaroid). Write the animal's breed, date of birth, and your name on the reverse side of the photographs.
13. Review your completed form for accuracy and completeness.
14. Submit the application form by mailing it to the specified address or ensure electronic submissions are sent correctly.
