- At the end of the form, calculate and enter the total score for the shift by summing the scores for all observed behaviours.
- Review the total score against the provided categories: 21 or less indicates normal behaviour, 22 to 28 indicates mild concern, 29 to 35 indicates moderate concern, and 35 or more indicates severe concern.
