Leukocyte esterase is an enzyme released by neutrophils and macrophages. A urine dipstick positive for this enzyme indicates pyuria (an increased number of leukocytes). Urinary tract infections including cystitis and urethritis are common causes of pyuria.

What Is It? Bacterial colonization in urine is high when the level of bacterial counts is elevated meaning the number of colonies of a single organism is higher than 100,000 per mL. If the bacteria level in your urine is high and it's causing physical symptoms, you have a symptomatic urinary tract infection (UTI).

Leukocyte esterase is a screening test used to detect a substance that suggests there are white blood cells in the urine. This may mean you have a urinary tract infection. If this test is positive, the urine should be examined under a microscope for white blood cells and other signs that point to an infection.

The urinalysis is a set of screening tests that can detect some common diseases. It may be used to screen for and/or help diagnose conditions such as a urinary tract infections, kidney disorders, liver problems, diabetes or other metabolic conditions, to name a few.

A positive urine dipstick has a great chance of being false positive. The average of the positive predictive value in the studies shows 61 percent. However, a negative urine dipstick seems to be more reliable, in which the negative predictive value shows an average of 83 percent.

Normal urinary proteins values are less than 150 mg/d and are undetectable using urinary dipstick. The urinary dipstick only detects the presence of albumin and no other proteins. When urinary protein values exceed 300-500 mg/d, the dipstick test result becomes positive.

Check the urine dipstick expiry date. Remove the testing strip from the container. Fully submerge the test strip into the urine sample. Remove the test strip and wipe away any excess urine. Ensure the test strip remains horizontal to avoid cross contamination.

Collect urine mid-stream using a collection container. Dip the strip into the sample for no longer than 2 seconds and remove any excess by wiping the test strip on the side of the container. Read the results after 60 seconds (for testing for Leukocytes, read after 90-120 seconds).
