To receive a headstone, marker, or medallion, the eligible applicant must submit a VA Form 40-1330, Claim for Standard Government Headstone or Marker or VA Form 40-1330M, Claim for Government Medallion to the VA along with proof of military service, to request a Government-provided headstone or marker.

All veterans with other-than-dishonorable discharges are eligible for free burial in a national VA cemetery. Space is limited; the VA recommends you request a pre-determination of burial eligibility to avoid any delay when the time comes. Most states have their own veterans cemeteries.

In regard to grave marker costs, you can expect to pay a few hundred dollars for a baseline option and $1,500 to $3,000 for a standard option. As you get up to high-end markers, they can easily cost as much as $20,000.

Quantico, VA 22134-3903 IMPORTANT: If you are requesting a replacement headstone or marker due to an incorrect inscription, damage, or non-receipt, please explain in Block 33 Remarks. To submit multiple application packages: Fax one application package (application plus supporting documents) at a time.

General Information. The Department of Veterans Affairs (VA) furnishes upon request, at no charge to the applicant, a Government headstone or marker for the unmarked grave of any deceased eligible Veteran in any cemetery around the world, regardless of their date of death.

While both a gravestone and a grave marker share a similar purpose in showing who is buried in a specific spot, a headstone is raised above the ground, while a grave marker lays flat against the ground.

The VA provides free headstones or markers for the unmarked grave of any deceased eligible Veteran in any cemetery around the world.

The VA will provide each deceased veteran with an upright grave marker in granite or marble, or a flat grave marker in granite, marble, or bronze. Upright grave markers are two-to-three-inch thick slabs which stand vertically at the head of a grave; they look like typical headstones.

An eligible spouse or other family member buried in a national cemetery, state or tribal Veterans cemetery, military post cemetery, or military base cemetery may be eligible for a headstone or marker. They may still be eligible even if they die before the Veteran.

Many variables affect the price of a granite memorial, such as the size of the marker and type of granite used. Our standard flat grave markers cost around $1,000, smaller garden memorials for home memorialization begin at 300, and upright headstones start at $1,200.
