8. Calculate the proposed CityFHEPS monthly rental assistance by subtracting the amount from step six from the amount from step seven.
9. In the landlord and room rental information section, provide the landlord’s name and address, as well as the address of the room being rented.
10. Complete the clearances completed section by indicating whether the apartment is subsidized housing. If yes, specify the subsidy type.
11. Review the landlord room rentals checklist and ensure all items are marked accordingly to confirm that requirements are met.
12. For the payment request, indicate the requested payment amount for the first four months of rent and name the landlord or authorized payee to whom it should be payable.
