- In Section 5 – Backflow Device Information, indicate the type of device by selecting the appropriate option (RP, RPDA, DCVA, DCDA). Fill in the serial number, size, hazard level, manufacturer, model number, and specific location of the device. Specify the device orientation and type of test (annual, new installation, or replacement). If applicable, provide the building permit number.
- Proceed to Section 6 – Backflow Testing. Indicate whether it is a test or re-test and, if necessary, record any repairs made after a failure in the remarks section. Fill in the results of the testing, including various pressure differentials and test results (pass/fail) along with the test date.
- Once all sections are complete, review the information for accuracy, and save your changes. You can then download, print, or share the completed form as needed.
