6. If applicable, fill out the Tribal Lands Lifeline section. Confirm your participation in designated programs for additional support and provide the necessary documentation.
7. Ensure all provided information is certified under penalty of perjury. Initial each numbered statement and provide your signature and date at the bottom, acknowledging the accuracy of the information you provided.
8. After completing the application, save your changes and download a copy for your records. Then, print and mail the completed application along with the consent to credit check to the given address.
