- In Column 5, document the name and license number of the supplier you acquired the product from.
- Enter the seller’s FEIN in Column 6 to further identify the source of your product.
- Column 7 asks for the date received. Ensure that you input the date accurately for bulk transactions.
- In Column 8, provide the document number associated with your transaction. For un-consolidated rack purchases, this number will be issued from the terminal.
- Finally, in Column 9, state the billed whole gallons sold — rounding to the nearest whole number for accuracy.
- Once completed, review the entire form for accuracy. You can then save your changes, download, print, or share the form as needed.
