4. If your group is a SAH or CG and signed an agreement with the Minister less than two years ago, include a completed and signed Settlement Plan (IMM 5440).
5. For a G5 group with more than five members, replicate the checklist and ensure all required information is completed for each member including their proof of Canadian citizenship or permanent residency.
6. For every group member, provide the necessary documentation such as the Financial Profile (IMM 5373B) and proof of funds as applicable.
7. If applicable, attach the optional completed and signed Use of a Representative form (IMM 5476).
8. Review all sections of the form carefully to make sure all required fields are filled out correctly before proceeding.
