- Provide details in the Water Pollution Information section including outlet location, distribution, and estimated average values for BOD/TSS concentration and discharge rates.
- Fill in the Flow Meter Information where you will indicate if a flow meter exists and its type, along with the method used.
- Address the Information on the Wastewater Treatment System. Indicate whether a treatment system exists, the capacity, and details regarding primary and secondary treatments.
- Attach a vicinity map showing the location of the establishment relative to significant landmarks.
- Finally, certify that the information provided is true and correct by entering the date and including the names and signatures of the PCO and CEO.
