2. Begin with the General Information section. Here, you will need to enter the name of the establishment or plant, establish the code, and provide the Tax Identification Number (TIN). Fill in the plant address with the street name, barangay, city or municipality, province, and telephone and fax numbers.
3. Indicate the type of industry and provide the name of the Pollution Control Officer (PCO), along with the accreditation date.
4. In the Employment and Operation Information section, enter the total employment number in your factory. Specify the production time and the number of hours, days, and months of operation.
5. Provide details about your sources of water and wastewater generation. Include each generating process, monthly average volume of water supply, and attach relevant water bills as required.
6. For Dwelling Units Information, fill in the total floor area, number of bedrooms, number of guests per year, and total area for dining units.
7. In the Product Information section, list the products manufactured. For each product, include the product name, annual production from the previous year, and the type of process (batch or continuous).
8. Provide details in the Water Pollution Information section including outlet location, distribution, and estimated average values for BOD/TSS concentration and discharge rates.
9. Fill in the Flow Meter Information where you will indicate if a flow meter exists and its type, along with the method used.
10. Address the Information on the Wastewater Treatment System. Indicate whether a treatment system exists, the capacity, and details regarding primary and secondary treatments.
11. Attach a vicinity map showing the location of the establishment relative to significant landmarks.
12. Finally, certify that the information provided is true and correct by entering the date and including the names and signatures of the PCO and CEO.
13. Save the changes, download, print, or share the form as necessary.
