This form should be submitted to the DOLE Regional/Provincial/Field Office as soon as possible. Page 1 should contain general information about the establishment and the number of workers affected. Page 2 should enumerate the names of workers affected, their addresses and contact numbers, position title and salary.

At present, employers may now submit its Establishment Report on Flexible Work Arrangement/Alternative Work Scheme, Temporary Closure, Retrenchment/Reduction of Workforce, and Permanent Closure via website http://reports.dole.gov.ph.

DEPARTMENT OF LABOR AND EMPLOYMENT Accomplish this form in two copies when filling notice of displacement of flexible work arrangements. The report is considered as dully filled when the complete list of workers affected is made part of the submission.

One valid ID and a notice of termination from the employer or an affidavit of termination of employment are required to secure a DOLE-issued certificate of involuntary separation. Once documents are evaluated and validated, the certificate will be issued the same day.

Getting the certification According to the guidelines, involuntarily separated employees, including kasambahay, may apply for certification at the DOLE Field or Provincial Office where their company or employer is located, or where the employees reside.

The applicant shall pay a registration renewal fee of Twenty-Five Thousand Pesos (P25,000.00) to the DOLE Regional Office. (b) Certificate of pending or no pending labor standards violation case/s with the National Labor Relations Commission (NLRC) and Department of Labor and Employment (DOLE).

This form should be submitted to the DOLE Provincial/Field Office as soon as possible. In the event of the implementation of FWA, TC, or retrenchment, the establishment shall accomplish and submit the RKS Form 5 2020.
