Follow the steps to complete the Amvets form online.

2. Begin on page 1 by entering your Squadron number, address, city, state, and zip code. It is important to ensure that all information is entered clearly and correctly.
3. Provide the Employer Identification Number (E.I.N.) of your Squadron, and include the name and contact information for the Membership Chairman. Ensure the phone number and email address are current.
4. On page 2, complete one line for each member being submitted. Include each member's full name, complete address, phone number, date of birth, email address, and indicate whether the member is a new applicant or a renewal.
5. After filling out the individual member information, include the total number of members submitted and the submitted by section, specifying the title of the person completing the form.
6. Prepare a check for the total amount due, calculating both the Department Dues and National Dues as specified in the instructions. Ensure that this check is ready to accompany the completed form.
7. Once the form is completed, make sure every section is legible and all information is accurate. Send one copy of the completed D & R Form to your State Vice Commander for Membership and keep one for your records.
