Follow the steps to complete the timesheet accurately.

1. Click the ‘Get Form’ button to access the timesheet and open it in your chosen editor.
2. Begin by entering your full name in the designated employee name section. Please use the format 'LAST NAME, FIRST NAME' and ensure that it is legibly printed.
3. Select your preferred payment method by checking the appropriate box: Direct Deposit, Pay Card, or Mail.
4. Indicate the type of assignment you are working on by checking either 'CONTRACT ASSIGNMENT' or 'TRAVEL ASSIGNMENT'.
5. Select your role by marking the corresponding box for RN, LPN, or CST/ORT.
6. Sign your name in the staff signature section to confirm the accuracy of the information provided.
7. Fill in the Client/Facility Name to specify the organization you are working with.
8. Record the days worked, including filling in the time in and time out for each day. Include lunch breaks if applicable.
9. Indicate the total hours worked for the week at the bottom of the section.
10. If applicable, choose 'Yes' or 'No' to indicate whether you worked as a charge nurse during any shifts.
11. Complete the sections related to on-call duties, noting the times as required.
12. Obtain the supervisor's signature for verification of your timesheet details.
13. Lastly, submit your completed timesheet by scanning or emailing it to travelpay@giftedhealthcare.com, or by faxing it to 888.254.6156 before the deadline of Monday at 12:00 pm.
