The Functional Assessment Interview Form (FAI; O'Neill et al., 1997) is a semi-structured interview designed to gather information about a behavior and the circumstances under which it occurs.

A simple email will do. “Dear IEP team, I am requesting that my child receive an FBA. I believe he/she has behaviors that are interfering with his/her ability to access and benefit from their education. They are: (and list them in bullet points).”

They go on to define functional behavior assessment and how to utilize it through 6 steps: Collect Data, Develop Hypothesis, Direct Observation, Behavior Support Plan, Implement Scripts, and Evaluate/Redesign.

Parents of a child with an individualized education program (IEP) can request an FBA when their child's behaviors interfere with learning or when their child has a placement changed due to disciplinary reasons.

A person trained in the specific type of data collection and analysis should conduct the interviews and observations. Examples of people who can do an FBA are school psychologists, special education teachers, assistant principals, and principals.

An FBA is a process to collect and study data to better understand what may be prompting the behavior which occurs and to understand the function of the behavior. FBAs are also addressed in the Individuals With Disabilities Education Act (IDEA).

Family caregivers can request an FBA/BIP process any time there are concerns that a student's behavior is a barrier to their education. Families have the right to participate in all educational decision making for their students.

The short answer to the question “can you do an FBA without an IEP?” is yes. When you think about what is an FBA in school, it is just documenting behavior to figure out if there is something specific. Behavior can still be looked at whether a child has an IEP or not.
