Filling out the Fandirect Order Form is an essential step in ordering a fan for someone who is currently incarcerated. This guide will provide you with easy-to-follow instructions to complete the form accurately.
Follow the steps to fill out your Fandirect Order Form online.
- Click ‘Get Form’ button to access the Fandirect Order Form and open it in your preferred editor.
- Fill in the purchaser's information. This includes your name (last, first, middle initial), street address, city, state, and ZIP code. If your mailing address differs from your street address, please provide that as well.
- Enter your primary phone number and an optional secondary phone number. Additionally, provide your email address to ensure you receive confirmation regarding your order.
- Proceed to fill in the offender's information. Input the offender’s name (last, first, middle initial) and their TDCJ number.
- Select the unit where the offender is currently housed from the provided list. Make sure that the selected unit is eligible for the FanDirect Program.
- Once you have completed all required fields, ensure that everything is accurate. You can then save your changes, download the completed form, or print it out.
- Finally, send the completed form along with a $20.00 money order or cashier's check, made payable to TDCJ Commissary & Trust Fund, to the provided address for processing.
