- Explain your reason for using a nominee, and indicate whether they will be an assistance nominee or legal nominee.
- Specify the duration for which they will act as your nominee.
- Sign the form, authorizing the nominated person or organization to act on your behalf and acknowledging the privacy notice.
- If you cannot sign, a designated person can sign on your behalf, but they cannot be the nominee unless legally designated.
- Lastly, ensure the redress nominee signs the form, agreeing to their responsibilities and obligations.
