- Proceed to Part B. Current benefits. If applicable, provide the name and case number for any household member receiving Food Assistance Program, Family Independence Program, or FDPIR benefits. Remember that Bridge Card Numbers and Medicaid Numbers are not acceptable case numbers.
- Move to Part C. Student information. Fill out the details for each student from Pre-K through 12th Grade. Enter the last name, first name, birth date, and indicate if the student is homeless, migrant, runaway, or foster.
- In Part D. Total monthly household income, report the income for all household members excluding foster children. If you provided a case number previously, skip this section and proceed to sign. Otherwise, complete each income type listed, circling 'None' if applicable for each category.
- Add the total monthly household income from Part D. This total must reflect the sum of all reported income types.
- In Part E. Signature, sign the form if the income section has been completed. You must also list the last four digits of your Social Security number or check the box indicating that you do not have one.
- Finally, review all entered information for accuracy. Once everything is confirmed, save your changes, and you may download, print, or share the form as needed.
