Follow the steps to effectively complete the checklist online.

  1. Use the ‘Get Form’ button to access the checklist and open it in your preferred online document editor.
  2. Begin by filling in the manager's name and date at the top of the form. Ensure this information is accurate as it reflects the responsible parties for the inspection.
  3. In the ‘Examiner’ section, provide the name of the individual who will be conducting the inspection. This establishes accountability.
  4. For the Office of Supervisory Jurisdiction (OSJ) section, indicate whether the branch is an OSJ by selecting 'YES' or 'NO'. This helps categorize the branch for inspection purposes.
  5. For each representative, ensure that you indicate if the required documentation (FINRA and state licenses, U-5 forms, etc.) is on file by marking 'YES' or 'NO'. This is crucial for verifying compliance.
  6. Continue filling out the sections that cover various files, such as office records and customer files. Include details for each item listed, ensuring all ‘YES’ or ‘NO’ questions are answered accurately.
  7. As you proceed through the checklist, pay special attention to specific requirements under Correspondence, Advertising, and Sales Literature, making sure each relevant item receives appropriate approval.
  8. Complete the sections on complaints, office procedures, and office identification. Double-check that all necessary documentation for changes in address or account information is included.
  9. Review the recommended products section, ensuring all transactions are detailed and comply with regulatory standards, noting any significant transactions related to mutual funds and variable products.
  10. Finally, make sure to add any comments or notes you find pertinent in the provided space at the end of the checklist. Review the entire document for completeness and accuracy.
  11. Once all sections are accurately filled, save changes, and utilize options to download, print, or share the completed checklist as required.

Non-branch sales office means any business location of the broker-dealer identified to the public or customers by any means as a location at which a securities business is conducted on behalf of the broker-dealer which location is identified solely in a telephone directory line listing or on a business card or ...

A "branch office" is generally defined as any location where one or more associated persons of a member7 regularly conducts the business of effecting any transactions in, or inducing or attempting to induce the purchase or sale of any security, or that is held out as such.

An OSJ branch must have at least one on-site supervisor who is a qualified and registered principal with the firm. The main office of each firm is always considered an OSJ and has supervisory jurisdiction and responsibility over all the firm's non-OSJ branch offices.

In establishing a non-branch location inspection schedule, there is a general presumption that a non-branch location will be inspected at least every three years, even in the absence of any indicators of irregularities or misconduct (i.e., "red flags").

A “non-supervisory branch office” would be any location of the member that satisfies the definition of branch office, but does not qualify as a supervisory branch office or limited supervisory branch office. A main office of a member may qualify as a non- supervisory branch office.
