  5. After completing item 2, place your signature and the date in the designated area. Ensure you have attached all required documents before submitting the form to your employer.
  6. In section B, your employer will fill out their details and nominate a super fund if necessary. They need to sign and date this section when providing you the form.
  7. Once you return a completed section A to your employer, they will document the acceptance of your chosen fund in section C. This section should only be filled in after you have submitted your part.
  8. Employers must retain the completed form for their records for five years. Finally, you and your employer can save changes, download, or print the completed form as needed.
