- In Section D – Sewer Information, describe the location of monitoring/sampling and discharge points for wastewater. Detail the outfall numbers and corresponding monitoring point.
- Complete Section E – Wastewater Discharge Information. For non-categorical users, list your average and maximum wastewater discharge along with discharge types. If your facility is a categorical user, provide the applicable discharge flows for regulated processes.
- Proceed to Section F – Characteristics of Discharge. Report monitoring data for pollutants that are known or suspected to be present in your waste stream. Make sure not to leave any blanks.
- Fill in Section G – Treatment. Indicate whether your facility practices any form of wastewater treatment and describe any planned changes within the next three years.
- In Section H – Facility Operational Characteristics, specify whether the facility discharge is continuous or seasonal and provide additional comments if necessary.
- Finally, complete Section I – Non-Discharged Wastes, detailing any waste liquids or sludges generated that are not disposed of in the sanitary sewer system.
