If the student did not file a return, provide details about any employment, including the employer's name, the annual amount earned, and whether the IRS W-2 form is attached.
- Next, complete Section B for the dependent student’s parent(s). If the parents filed a 2015 income tax return, indicate whether the IRS Data Retrieval Tool was used or if an IRS tax return transcript is attached. If the parents did not file, provide employment details similar to the previous step.
- Also in Section B, check any applicable boxes related to child support payments or Supplemental Nutrition Assistance Program (SNAP) benefits received by anyone listed in Section A.
- Finally, sign and date the certification section at the bottom of the form. Both the student and at least one parent need to sign to certify that all information provided is complete and accurate.
