Follow the steps to fill out the District Provided Professional Development Form online.
- Click ‘Get Form’ button to obtain the District Provided Professional Development Form and open it in your preferred document editor.
- Log into your Michigan Online Educator Certification System (MOECS) account. If you need assistance with your login, please use the Login Assistance button.
- Enter each professional development activity you have completed in your MOECS account, utilizing the 'View Professional Learning' tab. Ensure that your entries do not exceed eight hours per day and include the following elements: descriptive titles, categories, and hours of training.

- Take a printout of the list of DPPD hours you have added to your MOECS account using your computer's print screen function. This printout is essential for the subsequent steps.
- Complete the educator section of the DPPD Form and attach the printed screenshot from the previous step.
- Submit the form, along with the attached printout, to your Principal or designated school representative for required verification and signature.
- Once you have obtained the necessary signature, keep the signed form and printout for your records. Be prepared to submit these documents to the Michigan Department of Education (MDE) upon request.
- If your Principal or school designee refuses to sign your form as verification of your hours, promptly delete the entries from your MOECS account.
- Finally, save any changes you have made to the form, and consider options to download, print, or share the completed document as needed.
