1. Press the ‘Get Form’ button to access the Moc Form and open it in the designated editor.
2. Enter the date of submission in the format dd/mm/yyyy at the top of the form.
3. In Section 1, provide the title of the CDM project activity or program of activities, such as 'India-FaL-G Brick and Blocks Project No.1', and its reference number.
4. Proceed to Section 2 to indicate whether you are adding a new project participant or changing the name of an existing participant. If selecting the latter, please also fill in the former name.
5. For each project participant, provide the name of the entity, complete address, and the country authorizing participation. Include the contact details of both the primary and alternate authorized signatories, ensuring to fill in necessary fields such as first name, last name, telephone, email, and specimen signature.
6. At the bottom of the form, ensure that one signatory from the nominated focal point completes the signature fields. Only one signature is required per focal point entity.
