- On line 8, enter the code that identifies the origin of the health coverage. Only one letter can be entered from the following options: A (SHOP), B (Employer-sponsored), C (Government-sponsored), D (Individual market), E (Multiemployer), or F (Other designated coverage).
- Move to Part II if you had employer-sponsored coverage. Here, complete lines 10-15 with details about the employer. If this part is left blank, you are not required to provide additional information.
- In Part III, complete lines 16-22, which pertains to the issuer or other coverage provider. Enter the name and contact details for the coverage provider, including the telephone number on line 18.
- Finally, in Part IV, list all covered individuals on lines 23-28. This includes their names, social security numbers (or other identifiers), and coverage details. Ensure to check column (d) if an individual was covered for all months, or fill in column (e) for specific months they were covered. If listing more than six individuals, refer to the continuation sheets.
- After completing the form, review your entries for accuracy, then save the changes. You can download, print, or share the form as needed.

Form 1095-B will report the months of MEC a Medi-Cal beneficiary received during the calendar year. DHCS will send your MEC information to the IRS and beneficiaries are not required to provide Form 1095-B to the IRS, if they chose to file their taxes.
