In the state of California, a death certificate must be filed within 10 days of the deceased's passing. The certificate must be signed by a licensed physician, coroner, or other authorized medical professional.

California law defines individuals who can obtain an authorized copy of a death certificate as: The registrant (person listed on the certificate) or a parent or legal guardian of the registrant. A child, grandparent, grandchild, brother or sister, spouse, or domestic partner of the registrant.

How much does is cost? o There is no processing fee to file a Physician/Coroner's Amendment.

​$26 amendment fee and includes one free copy of newly amended record.

o To amend Cause of Death and Medical Information in Items 101-125 (Fields 119- 125 are Coroner only fields), the certifying physician or coroner who originally attested to the death certificate must sign the form. Name of attesting physician can be found in box 115 of the California Death Certificate.
