  4. Following the report, a representative from Sedgwick CMS will contact you for additional information about your injury or illness.
  5. As a supervisor, ensure that you access the required forms from the Environmental Health and Safety (EH&S) website. If you cannot provide the Claim Form (DWC-1) to the employee within 24 hours, call the designated number immediately.
  6. Upon receiving notification of the injury, complete the Work Related Injury/Illness Form and provide the DWC-1 form to the employee for completion. Ensure they return it promptly if medical treatment is required.
  7. Fax the completed forms to EH&S and keep copies for the employee's records.
  8. If medical treatment is needed, contact the appropriate medical facilities immediately. Remember to follow up on any work status updates after each doctor’s appointment.
  9. If no treatment is needed, submit the OII (Band-Aid) form. If the employee later requests treatment, they must contact the Workers' Compensation Analyst to submit the DWC-1 form.
  10. Once everything is complete, save any changes, download, print, or share the form as needed.

The OSHA Form 300 is a form for employers to record all reportable injuries and illnesses that occur in the workplace, where and when they occur, the nature of the case, the name and job title of the employee injured or made sick, and the number of days away from work or on restricted or light duty, if any.

The OSHA 300 Log requires employers to check one of 6 boxes to categorize the injury/illness: (1) injury (2) skin disorder (3) respiratory condition (4) poisoning (5) hearing loss (6) all other illnesses. There are spaces to record days of job transfer or work restriction, as well as days away from work.
