6. Complete the Over-the-Counter (OTC) certification by specifying the purpose of the purchased items and certifying that they are for medical care.
7. Finalize the process by signing the Authorization section. Your signature confirms that the provided expenses meet the necessary criteria for reimbursement and are not covered by any other plan. Include the date of your signature.
8. Once the form is filled out completely, attach any relevant receipts and prepare to send it to Anderson/Bunbury at the address provided. You also have the option to fax it if necessary.
