8. List any learning or physical disabilities, allergies, self-medication requirements, and inhalers. This information is essential to ensure your child's needs are met.
9. Finally, provide the names of individuals who cannot pick up your child after religious education classes.
10. Sign and date the form, ensuring it corresponds with the grade your child is entering in September. Confirm the details are accurate before finalizing.
11. Once completed, you can save changes, download, print, or share the form as needed.
