1. Click the ‘Get Form’ button to access the application form online.
2. Select your desired membership type by indicating whether you are applying for an Ordinary Member, Associate Member, or Affiliated Member.
3. Affix a recent photo in the designated area provided on the form.
4. Fill in your surname and other name as requested, and include your Chinese name if applicable.
5. Enter your correspondence address, along with your office and mobile telephone numbers and fax number. Please ensure that your email address is also provided as it is a mandatory field.
6. List your academic and professional qualifications in chronological order, including the year obtained, the name of the qualifications, and the institution where you obtained them.
7. Document your relevant work experience in chronological order, detailing the year, hospital or institution, your position, and whether it was full-time or part-time.
8. Read and agree to the photo release clause, which allows AHKDN to take and use photographs of you in relation to AHKDN events.
9. Submit a crossed cheque for the applicable membership fee. Note the bank, cheque number, and include your signature and date.
10. Mail the completed application form along with the cheque to the designated councilor as specified in the instructions.
11. After submitting your application, keep track of your application status and any communications from AHKDN.
