- Complete Line 4 with the total tax collected, inclusive of any discretionary sales surtax from Line B.
- Deduct any lawful deductions in Line 5 and any applicable Department of Revenue (DOR) credit memos in Line 6.
- Calculate the total tax due and enter this amount in Line 7, factoring in any collection allowances or penalties from Line 8.
- Finally, enter the total amount due with the return in Line 9. This amount should be noted on the front of the form.
- Review all entries for accuracy, then provide your signature, the date, and a contact number.
- If a preparer assisted you, their signature and date will also be required.
