How to fill out the Framp online
The falls risk assessment and management plan, known as FRAMP, is an essential document designed to ensure the safety of patients at risk of falls. This guide provides a clear, step-by-step approach to filling out the FRAMP online.
Follow the steps to complete the FRAMP effectively.

- Fill in the patient’s surname using block print or an I.D. label, followed by their unique medical record number (UMRN), given names, date of birth (DOB), gender, address, postcode, ward, and telephone number.
- Indicate if the patient has been admitted or transferred from another ward, experienced a fall, or undergone medical deterioration. Select ‘YES’ or ‘NO’ as applicable.
- Conduct the initial screening by checking whether the patient has had a fall in the last 12 months, is unsteady when walking or transferring, has cognitive impairments, or has issues related to urinary or fecal frequency. Circle 'YES' or 'NO'.
- Document any individualized interventions that are in place and those that need to be executed. Include the name, designation, dates, and actions taken by staff members.
- Communicate risks and information to patients and carers as necessary. Ensure to document discussions and confirm understanding.
- Complete all relevant checks indicated on pages 2, 3, and 4, ensuring all sections for ongoing patient assessment and interventions are updated.
- Once all sections are completed, review the information for accuracy and save any changes made to the FRAMP. You may choose to download, print, or share the completed form as needed.
