USE THIS FORM TO REQUEST A HIGHER-LEVEL REVIEW OF A DECISION YOU RECEIVED. A Higher-Level Review is a new review of an issue(s) previously decided by the Department of Veterans Affairs (VA) based on the evidence of record at the time VA issued notice of the prior decision.

Use VA Form 21P-601 to apply for accrued benefits due the beneficiary but not paid prior to death. Each person claiming a share of accrued benefits must complete a separate VA Form 21P-601. Any available accrued benefits are payable to the first living person listed below.

Use VA Form 21-651, Election of Compensation in Lieu of Retired Pay or Waiver of Retired Pay to Secure Compensation From Department of Veterans Affairs (38 U.S.C. 5304(a)-5305) when it is necessary to obtain the veteran's or fiduciary's signature on the election or waiver.

However, a Veteran may, on his/her own initiative, notify VA of his/her receipt (or anticipated receipt) of drill pay, and whether he/she chooses to waive VA benefits or drill pay, by completing and submitting VA Form 21-8951-2, Notice of Waiver of VA Compensation or Pension to Receive Military Pay and Allowances.
