Liquid chromatography mass spectrometry accurately measures DOAC levels but is clinically impractical. Dilute thrombin time and ecarin-based assays using appropriate calibrators/controls provide an accurate DTI level. Anti-Xa assay using corresponding FXaI calibrators/controls provides accurate drug levels.

Direct oral anticoagulants (DOACs) are oral medications that specifically inhibit factors IIa or Xa. They are also known as new oral anticoagulants (NOACs) or target-specific oral anticoagulants (TSOACs). DOACs are the preferred name ing to the International Society of Thrombosis and Haemostasis [1].

In general, the DOACs are safer and more effective than , especially when it comes to serious bleeding. DOACs cause half as much life-threatening bleeding than . They're also more convenient than because they don't require frequent blood monitoring and can be given safely in fixed doses.

At standard doses DOACs are safe and effective for most patients and lab monitoring is not required for routine use. If the time of the last dose is known, an estimate of current anticoagulation effect can be obtained by measuring the number of half-lives that have elapsed in those with normal renal function.

DOACs thin the blood within a day; once stopped, the anticoagulation effect wears off quite rapidly, within 24 to 48 hours. It can take days to weeks for to thin the blood in the correct range, and at least three to five days before the blood is no longer thinned after stopping .

What are DOACs? The term DOAC (pronounced 'DOH-ack') stands for 'Direct Oral AntiCoagulant' and refers to a group of new anticoagulant medications. An anticoagulant is a medica- tion that either treats or prevents blood clots, often called a 'blood thinner'.

Required Baseline clotting screening. Body weight. Full blood count. Liver function tests. Serum creatinine (for creatinine clearance) Urea and electrolytes.

The assay measures drug levels based on its bioactivity as an inhibitor of Factor Xa (anti-Xa assay method). Measuring bioactivity will allow dose adjustment to prevent too much anticoagulant effect and risk for bleeding, or no anticoagulant action and inadequate therapy.
