- Identify the purpose of the expenditure by circling one of the options: Purchase, P.O., Service, or Other. If you select 'Other,' please provide a brief description.
- Ensure that the goods or services are specific and clearly outlined. List each item along with the corresponding vendor's name.
- For each item, enter the amount in the ‘Amt.’ field next to the goods or services listed.
- Once all items have been listed and amounts provided, sign the form as the Club/Organization Representative.
- Make sure to provide a name for the 'Check Payable to:' section.
- Obtain the Faculty/Advisor signature in the appropriate space.
- Finalize your submission by saving any changes made, and then you can download, print, or share the completed form as needed. Remember to attach a copy of the meeting minutes for the organization where the expenditure was discussed.
