10. If your event will feature live animals, provide the type of animals and information about the company including their address, permit number, contact person, and phone number.
11. Compile a list of food vendors and caterers that will provide food to the public, including details such as their names, addresses, and phone numbers.
12. Review all entered information for accuracy and completeness. Finally, ensure to sign the application and provide the date, as well as printing your title.
