2. Begin by rating how easy the information given by your trainer was to understand. Circle a number from 1 (extremely difficult) to 7 (very easy) that best describes your experience.
3. Next, rate your trainer's friendliness and helpfulness by selecting a number from 1 (extremely unhelpful) to 7 (very helpful).
4. Indicate your confidence level in making the trip you were trained on by yourself by circling a number between 1 (not confident at all) to 7 (very confident).
5. Respond to the open-ended question asking why you feel that way. Provide a brief explanation in the space provided.
6. Answer whether you have made the trip by yourself with 'yes', 'no, will not be making it', or 'no, but will make it later'.
7. If you answered 'yes', indicate how many times you have made the trip since your training and what you used this trip for by ticking all that apply such as shopping, work, appointments, other, social, or PTO.
8. If you have not made the trip, provide reasons in the specified space.
9. Rate the printed materials you received during the training by circling a number from 1 (very inadequate) to 7 (appropriate and helpful).
10. Decide whether you would recommend travel training to a friend or family member by selecting 'yes' or 'no'. If 'no', please explain why.
11. Rate your confidence in planning a trip on public transport to a different location, using the same 1 to 7 scale.
12. Finally, provide any additional comments you may have about the travel training program.
13. Once you have completed the form, save your changes, download or print the document, or share it as required.
