- Begin by filling out Section A: General Information. Enter your Participant ID, study visit number, the version of the form, the date of your interview, and the initials of the interviewer.
- Move to Section B, which focuses on your pain over the past week. Question B1 asks if you have experienced pain outside of everyday types of pain. Respond accordingly.

- Inquire about treatments received in the past week for your pain in Question B7. Specify any medications or treatments you have received.
- Proceed to Question B8, indicating the percentage of relief you have experienced from the treatments or medications provided to you.
