7. For the Varicella vaccine information, enter the dates of the two doses received or provide positive serological proof of immunity.
8. Complete the influenza vaccine section by entering the date of the flu vaccine received annually.
9. Finally, the healthcare provider or institutional representative must fill out their details and sign the form to validate the information provided.
10. After all sections are completed, save your changes, and either download, print, or share the completed form as needed.
