  10. In the Approximate Current Asset Allocation section, provide the percentage allocations that total 100% for your assets outside of Chou Funds.
  11. Indicate your investment objective by selecting one of the options provided regarding safety, income, balanced, or growth.

  13. Determine if a third party is instructing you regarding your account and provide their details if necessary.
