1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Begin by filling out the Student Applicant Information section. Provide your student ID number, last name, first name, current address, city, state, ZIP code, home telephone number, cell phone number, and email address. Ensure all details are accurate to avoid processing delays.
3. Complete the Emergency Contact Information section. This includes the last name, first name, telephone number, relationship to you, and current address of your emergency contact.
4. Indicate your Enrollment Status by specifying the number of credit hours you are enrolled in for the semester and your program of study. You will also need to state how many semesters you have completed at Atlanta Technical College.
5. In the Loan/Grant Information section, select the reasons for your application. This includes school expenses, transportation assistance, housing assistance, and books/supplies. Clearly specify the total loan amount you are requesting, keeping in mind the maximum allowed is $350.
6. Complete the Student Deduction Authorization by indicating the amount you authorize to be deducted from your PELL/SEOG award or other surplus funds. Initial the section to confirm your understanding of the repayment responsibility.
7. Review the Information Terms & Conditions section to ensure you are aware of the stipulations and requirements of the program.
8. Prepare your supporting documents: a typed letter explaining your request, a completed household budget form, a copy of your government-issued driver’s license or ID, and any additional documentation to support your loan request.
9. Sign the application form in the designated area and date it. Ensure your signature matches the name provided in the Student Applicant Information section.
10. Submit your completed application and all required documents to the designated Drop Box at the Security Desk in the Cleveland Dennard Conference Center. Remember that applications submitted elsewhere will not be accepted.
11. After submitting, check the status of your application via email five business days later if you have not received a notification from the ATC Foundation.
