- Count and enter the number of commercial motor vehicles you operate in the U.S. Specify whether they are owned, leased, or rented.
- Summarize your driver information, including the total number of interstate and intrastate drivers. Specify how many hold a valid CDL.
- If applicable, provide the names and titles of company officers or partners and ensure to enter the certification statement appropriately.
