The DOT physical requires drivers to have 20/40 vision with or without the use of corrective lenses. This means you must be able to see at least 20/40 with both eyes, or at least 20/40 with either the right eye or left eye. Those with monovision may be disqualified from getting a DOT medical card.

You may certify online if you can upload an image of your D.O.T. Medical Card. You may use a scanner or smart phone to create the image.

If you require a medical fitness certificate as a required document in admission or registration process, then you should visit any nearby hospital and ask them that you require a medical certificate. The doctor in-charge will take some easy tests and will fill your form ingly.
