- Fill out the ‘Communication Skills’ section by assessing the student’s ability to initiate conversations, follow directions, and express feelings. Provide ratings and comments as needed.
- Next, address the ‘Academic Skills’ section, evaluating the student’s responses to teachers, time management, and basic academic abilities. Rate and comment as required.
- In the ‘Recreation and Leisure Skills’ section, observe the student’s ability to choose activities, turn-taking, and safety awareness. Complete the rating and comments.
- Continue to the ‘Community Participation Skills’ section to assess knowledge of community resources, travel skills, and social appropriateness. Document your observations with ratings and comments.
- Finally, evaluate the ‘Work and Work-Related Skills’ by observing task completion, schedule awareness, and work habits. Ensure you provide ratings and comments for this section.
- Once all sections are completed, you can save your changes, download, print, or share the completed Systematic Observation form as needed.
